Mystic Healthcare, a 100 bed SNF located in Mystic, CT., is currently seeking a Physical Therapist to join our team of healthcare heroes. Responsibilities Review request for physical therapy and physicians' orders Completes screening Interviews residents to determine treatment type and schedule therapy Participate in the development of resicent assessments (MDS) and care plans Brief resident of procedures involved in pt and prepare necessary equipment Assist in transporting residents to and from therapy Determine proper equipment usage, applicantion and body position, and make adjustments as necessary in the administration of treatments Ensure that all therapist notes are informative and descriptive of the care provided Consult with physician/staff as necessary Must have an unencumbered Physical Therapy License
