Key Responsibilities:

• Patient Assessment: Conduct comprehensive health assessments, including monitoring vital signs and identifying any changes in patient conditions.

• Care Planning: Develop and implement personalized care plans tailored to each patient's needs and medical conditions.

• Medication Administration: Safely administer prescribed medications and monitor for potential side effects.

• Wound Care and Medical Procedures: Perform advanced nursing tasks such as wound dressing, IV therapy, catheter care, and injections.

• Health Education: Educate patients and families about disease management, medication use, and healthy lifestyle practices.

• Emergency Response: Act promptly in medical emergencies, providing first aid or resuscitation if necessary.

• Collaboration: Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care delivery.

• Documentation: Maintain accurate records of patient care, including progress notes, treatment plans, and updates.

• Infection Control: Adhere to strict infection control practices to protect patients and caregivers.

• Emotional Support: Provide compassionate care and emotional support to patients and their families during difficult times.

Requirements

✅ Certification in nursing or healthcare assistance

✅ DOH license or eligibility letter (mandatory)

✅ Experience in home care or elder care (preferred)

✅ Strong interpersonal and patient care skills
