Social Worker MSW - Home Health at Hospice and Home Care of Snohomish County
Schedule: Day Shift, 8:00 am – 4:30 pm, M-F

This position is responsible for providing referral services to home care clients, families and caregivers, addressing issues such as adjustment to illness/prognosis, care system issues, community resource issues, and insurance/financial issues. The MSW develops and implements a plan of care in collaboration with the Home Care team providing care to the patient.

Required qualifications:
Master's Degree in Social Work.
Washington Advanced Social Worker License upon hire; or
Washington Advanced Social Worker Associate License upon hire; or
Washington Clinical Independent Social Worker License upon hire; or
Washington Clinical Independent Social Worker Associate License upon hire.
National Provider Identifier (NPI) number within 30 days of hire.
1 year experience in a health care setting.
